Checklist

In order to process your loan request, we will need the following information:

1. Completed Consumer Loan Application.
2. A copy of your driver's license.
3. For Home Equity Loans or Lines of Credit, open and review the brochure.
4. Copy of your 2 most recent paystub(s), or copies of your last 2 years of income tax returns if self-employed.
5. Name of your Homeowners Insurance Agent.
6. W-2
7. Mortgage Statement
8. Current Bank Statement
